Year 8 - Your child’s Human Papillomavirus (HPV) vaccine is due.
Dear Parent/Carer,
HPV vaccination is part of the National Immunisation Programme and is offered in schools. The vaccine protects against 9 types of HPV related cancers and genital warts. For more information, including possible side effects please visit: HPV vaccine - NHS
How to consent: We are coming to your child’s school Co-op Academy Leeds on 12th May 2026, please ensure that you have submitted a form even if you do not consent, by 3pm, of the PORTAL CLOSURE DATE 11th May 2026.

Important: If you do not wish your child to receive the vaccinations, you still need to complete the “refusal section” on the form so that we understand your wishes. If we do not receive a consent or refusal form, your child may be given an opportunity to consent for themselves on the day after being assessed by a nurse as Gillick Competent to do so. Gillick competence is where a child is believed to have enough intelligence, competence and understanding to fully appreciate what's involved in their treatment.
Providing you enter a correct email address, you will receive email confirmation when your child has received their vaccination, including information and advice about common side effects your child may experience.
If you change your mind regarding consent or there are any changes to your child’s health following submission of the form, you must contact us on the above telephone number.
